Fill out the Health Check below if you have any COVID-19 symptoms or exposure.
- Stay Home if you have any COVID-19 symptoms
- Inform your professor(s) and/or campus employer
- Fill out the Health Check, which triggers campus response, including Mt. 91直播 Contact Tracing
- Do not come to campus until you have been cleared by Mt. 91直播 Contract Tracers or a medical provider
